Community Dental Program
 

Our Community Dental Program serves intellectually disabled individuals over the age of 21 from the community, who are referred by their Case Managers/Support Coordinators.  The Community Services Boards of Chesapeake, Norfolk, Virginia Beach, Hampton/Newport News, Portsmouth, Western Tidewater, Colonial, Eastern Shore, and Middle Peninsula/Northern Neck are within our catchment area.

 
We offer general dentistry services at no charge (i.e. exams, cleanings, x-rays, fillings, and simple extractions).  Crowns (caps), bridges, dentures (partial and/or full), root canals, or complex extractions requiring an oral surgeon are not included.  General anesthesia is offered, if necessary.  We are unable to accommodate emergency referrals at this time.

To refer clients who are cooperative for dental services please mail in:

  1. The Health Examination Questionnaire.
  2. The signed Informed Consent for Routine Dental Care.
  3. Copy of the Authorized Representative appointment letter or Legal Guardian papers, if applicable.
  4. Copy of Individual’s photo ID for identification purposes.
Once the completed paperwork is mailed in, the contact person will be called and an appointment will be scheduled on a first come basis.
 
To refer clients who are uncooperative for dental services and may need general anesthesia please mail in:
  1. The Health Examination Questionnaire.
  2. The signed Informed Consent for Routine Dental Care Using General Anesthesia.
  3. Copy of the Authorized Representative appointment letter or Legal Guardian papers, if applicable.
  4. Copy of Individual’s photo ID for identification purposes.
The Anesthesiologist will review the Health Evaluation Questionnaire and the contact person will receive a letter ordering the pre-appointment lab work and any additional requirements.  Prior to scheduling the general anesthesia appointment, results will need to be mailed or faxed in for the Anesthesiologist to review.
