03/16/2022

Starting Sunday, March 20th, off campus visitation and overnight visitation options will resume. Any visits impacting medications will require 3 days advance notice. This is subject to change on short notice pending the community and campus COVID-19 positivity rates.

 

02/11/2022

Starting Sunday, 2/13/22, We will be resuming visitations. Also we are asking families to contact Reception at (757) 424-8240 to schedule visits to avoid overcrowding in the homes.  More than one family may visit at the same time in one home but the visitation must take place in separate spaces, i.e. one family at the Gazebo and one family on the screened porch in the home. 

We will be providing an updated visitation reminder to the families which include wearing a surgical mask at all times, completing hand hygiene before engaging with the Individual, to keep their visits only with their loved one and make attempts to social distance with everyone in the home.

11-23-2021...

Starting Wednesday 11/24/2021 at 12:00 PM, SEVTC will resume on-campus visitation. 

All visitors must report to Building 1 at the Reception area for screening PRIOR to visiting in the House.  A mask is required to be worn, before entering the Reception area.  The screening will include a temperature check, completion of a screening form, and ensuring a surgical mask is available for use in the Houses.  All visitors must wash their hands or utilize hand sanitizer before the visit, and practice social distancing.

All visits must be scheduled in advance, scheduling can be done by calling the main number at 757-424-8240 and will be limited to a 1 hour timeframe to allow visitation time for everyone.  The visit may be extended by 1 additional hour, if there is no other visitor scheduled for that House.

There will be only two (2) visitors per Individual allowed in the House at one time; therefore, requiring advance scheduling.  This will allow the House to ensure Infection Prevention and Control measures are adhered to before and after visitation.

Visiting hours will be 7 days a week from 9:00 a.m.-11:00 a.m. and 1:00 p.m.-5:00 p.m. and limited to 1 hour.

Visitation locations may occur as follows: the Family Room in the House, Individual's room, front porch, screened-in-porch, designated visitation area in Building 1, the Gazebo, or personal vehicles.  Movement around the Facility or the House is not allowed.

At this time, there will be no off campus visits

Visitation will not be allowed for homes currently in quarantine.

SEVTC will be implementing visitor restrictions effective 9/3/21 in accordance with DBHDS directive issued on 9/1/21 . COVID-19  community prevalence is being monitored. Visitation updates will be provided on this site and authorized representatives/guardians will be notified of any changes. Stay safe

5-03-2021...

Due to the number of  COVID-19 cases on campus, we feel the safest thing to do is  continue with the suspension of visitation at this time.  We sincerely apologize for this inconvenience and happy to coordinate a tele-visit until we can resume in person visits.     Our current quarantine is slated to end on May 17th assuming no additional cases arise.

This decision is not taken lightly and I want to continue ensuring the safety of everyone on campus.   The community prevalence remains around 7.5% and we continue campus wide weekly testing of all employees. 

4-22-2021...

We have temporarily restricted visitation for 14 days due to recent positive cases for employees (3). We currently have 2 homes on quarantine as a precaution.  We are working closely with the Health Department and will continue to provide updates related to visitation. Community prevalence is still high in our area so please remain diligent with your infection control practices

4-15-2021...

We want to remind you about some ways to help to protect you and your family from getting sick:

 v  Stay at home if you have symptoms

v  If children are sick please do not send them to school

v  Frequent handwashing with soap and water or using alcohol-based hand cleaner

v  Use tissues to cover mouth and nose when coughing or sneezing and dispose of the tissue in a wastebasket. If you do not have a tissue and need to cough or sneeze use your upper sleeve, not your hands.

v  Stay at least 3-6 feet from someone who appears to be ill.

v  Avoid touching eyes, nose, and mouth

v  Get plenty of rest

v  The symptoms of COVID-19 can range from mild to severe and may appear 2-14 days after exposure.

o   Fever

o   Cough

o   Shortness of breath

v  Older adults and people with underlying chronic medical conditions seem to be at higher risk for more serious disease course with COVID-19

v  Call your Doctor if you develop symptoms and have been in close contact with a person known to have COVID-19 OR have recently travelled from an area with widespread or community spread of COVID-19

IMPORTANT INFORMATION REGARDING THE CONSENT FORM PROCESS FOR ON-SITE COVID-19 VACCINATION CLINICS.

OVERVIEW:

The process and requirements for COVID-19 vaccination consent have been finalized and are now available to share externally with facilities. The attached guide outlines the various versions of consent forms as well as the requirements for each method of obtaining consent to receive the COVID-19 vaccine. This is for the CVS Pfizer vaccine and for the DBHDS provided Moderna vaccine.

The CVS Pfizer Vaccine information is listed first.

 Versions of Consent Forms:

  • COVID-19 CVS Vaccine Consent Form: *PREFERRED* physical, triplicate form shipped directly to facilities (alone or with RP cover sheet)
  • Electronic, editable PDF (two options):
    1. COVID-19 Vaccine CVS Intake Consent Form
    2. COVID-19 Vaccine Consent Responsible Party Form (abridged)

 --Requirements for CVS Clinics:

  • Three (3) copies of signed, completed consent forms (whether it’s gathered electronically or paper form).
  • If a responsible party completes the electronic cover sheet, we need three (3) copies printed, to attach one to each copy of the physical COVID-19 Vaccine Consent Form.
  • Two (2) complete sets of consent forms will be required for each individual participating in the vaccination clinic – one set for the first dose/clinic and a second set for the second dose/clinic.
